A shared decision making tool for patients suffering from respiratory disorders
"So Hum" is a holistic tool for patients suffering from Chronic Respiratory Conditions to manage their condition better and predict trigger events.
Patient interface - to capture self reported symptom trackers, patient journal/diary on aggravation/relief, access to wearables
, external triggers (climate, weather, travel history, allergy), medication adherence etc Doctor interface - to capture the patient history, vitals, examination findings which are clinically validated by a physicians
The above two tools helps monitor the patient's condition round the clock with optimum continuity of care and a feedback loop between the physician & the patient (+ caregiver if necessary) on a need basis from either stakeholder. This helps arrive at a "ResQ Score" based on clinical parameters (shared by the physician) & self reported parameters (shared by the patient) which results in shared decision making on the treatment, medication etc - thus leading to a better quality of life for the patient with the help of a patient-centric tool which is operated by both the physician and the patient.
