Challenge Data Core - There is a need of central repository for patient data that includes all details like:

1. The entire history of the patient diagnosis, prescription, hospital visits, admissions, test reports etc.
2. Having it at one place will enable patient as well as the health professionals retrieve data at run time.
3. Privacy and security of data should be also taken care of i.e. the access to patient data will be with permission of the patient only.

Challenge My Journey - 1. Patient should be able to search for experts/specialists in different areas . 2. Patient should be able to search for similar patient going through the same journey. 3. Patient should be able to share the experiences. 4. Patient need to to understand the journey of the health issues/diagnosis they are identified with. 5. One patient should be able to communicate with other patient.
